ORDERING INSTRUCTIONS, PRICE LIST & ORDER FORM
1. Type in the date, your name, address, phone numbers and e-mail address in
the boxes below and indicate if the address is a business or residence.
2. Print out the order form on your printer.
3. Fill in the picture numbers as you look
through the images and then fill in the sizes and quantity that
you want. Select which shipping carrier you want and enter the
amount for shipping, sub-total and the tax and total amount due. If you
are paying by MasterCard or VISA, enter the card number, expiration date
and the name listed on the card.
4. Mail the completed form with your payment to my mailing address below.

STATE SALES TAX is 8.25% Tax for Texas Residents.
Orders
shipped out of state are tax exempt.
SHIPPING:
We mainly ship USPS Priority Mail with delivery confirmation.
Shipping charges start at $ 6.00 but may be higher depending on
the size of the order.
If you prefer Federal Express, the base shipping charge is $ 30.00 per order shipped within the USA. For shipment outside the USA, please call or send an e-mail for a summary of charges.
